The Safety & Learning Team hosted the South Region Members Networking Forum on Tuesday 25th March 2025.
The presentation kick starting this our second Forum for the South was by Justine Sharp and Abigail Fogarty from NHS Resolution’s Safety & Learning Team in London. Their presentation covered an overview of the Learning from staff claims relating to exposure of substances hazardous to health report published January 2025. Abigail gave focus to how the System Engineering Initiative for patient Safety (SEIPs) is being adopted by the Safety & Learning Team to support members with learning from claims within a systems approach.
We were also joined by Trudy Sexton, Patient Safety Manager from Devon Partnership NHS Trust, marking the very first member led presentation at the South Forums. Trudy shared the response of the Trust to variations observed when undertaking Duty of Candour across the Trust and their progress in standardising and improving Duty of Candour conversations with the use of Relational Co-ordination which was supported by the Trust Patient Safety Incident Response Framework (PSIRF) implementation core group. Here we share one of the images from the slides presented as this relates to the identification of barriers to how people may have different perspectives.

A networking opportunity for members was provided with smaller breakout groups focusing on introduction and discussion on the presentations provided.
